1 What personal information is collected and why?

- Name and date of birth

- Contact information (phone number, email, address)

- Ethnicity

- Emergency contact

This information is collected in order for us to provide physiotherapy treatment and contact you regarding your injury. We ask for your ethnicity in order to provide culturally appropriate services. Our access to your personal information is limited only to what is necessary.

Do you ever share my information? 

Only with other medical practitioners involved in your care, with ACC and any medical insurance companies that may be providing funding for your physiotherapy. In some cases, we may be required to disclose your information by law.


Before doing so, we always inform you and ask for your consent (or the consent of your guardian if under 16) and confirm the details are accurate, relevant and up to date.

What are my rights regarding my personal information that you hold? 

You have the right request a copy of all the information we hold on you and ask for any corrections if necessary.

How do you secure the information I provide?

All patient records are stored online via Cliniko. If any manual forms are completed, these are scanned and uploaded to your patient profile online and the original destroyed. 

Two factor authentication is required to access data stored on Cliniko. Patient information and treatment logs are accessed by the physiotherapists and administrative manager only when necessary.

As you use this website, certain technical information may be collected without your active participation using technologies such as internet protocol addresses, cookies, internet tags, and navigational data
collection tools. 

 

Such information may include:

- server addresses
- top-level domain names
- dates and times of website visits
- pages accessed
- documents downloaded
- links clicked on

- previous websites visited
